At each follow-up visit we ask our patients and families to provide updated written information on the impact of medication therapy, current functional and health concerns, and difficulties or side effects with medications. We also ask patients and families, to complete evidence based behavioral rating scales. The appropriate forms can be completed before the visit and submitted at the visit or sent to our office by email.
